You will need to leave your name, address, date of birth, telephone number, patient ID (if known), names and doses of drugs (spelling difficult names when necessary) and specify to which chemist you would like your prescriptions sent.
Computer generated prescriptions have a tear-off segment on the right hand side containing a complete list of current medications. Please keep only the up-to-date list and discard any previous lists each time you obtain a prescription.
Medication will be ready for collection from your nominated chemist 72 hours after ordering.
You can also request a repeat prescription online.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
